Job Summary
Respond to clinical referral information with care, assessing client eligibility for specialized hospice services. Partner with the Clinical Director to guide care plans, delivering hands-on nursing support to ensure patient comfort and quality of life. Educate patients and families to encourage active participation in personalized care goals while sharing call duties to provide support when it matters most. Work per diem Monday through Friday, 8 am to 4:30 pm, serving patients in the Wheaton, Warrenville, and Lisle areas.
Required Qualifications
- Hold current unencumbered license as a Registered Nurse
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Must be computer proficient in typing and various programs, including background in EMR
- Possess and maintains current CPR certification if required by state
- Current driver's license
- Vehicle insurance
- Access to dependable transportation
