Job Summary
RN PRN Case Manager coordinates and maintains a comprehensive plan of care for hospice patients and families across home, long-term care facilities, or assisted-living facilities. Responsibilities include reviewing and updating the care plan and physician orders, educating patients and family members about the disease process, available palliative interventions, the dying process, and safety practices, and serving as the coordinator for patient and family interactions with Tidewell Hospice throughout the hospice experience. Requires nursing licensure in Florida; BSN/MSN preferred; CPR certification; minimum one year of direct patient care in an acute-care setting; hospice RN experience preferred; CHPN encouraged within three years of employment.
Required Qualifications
- Graduate of approved nursing program
- Current unrestricted Florida RN license
- CPR certification
- BSN or MSN preferred
- Minimum one year experience in an acute-care hospital environment providing direct patient care
- Previous hospice RN preferred
- CHPN certification encouraged within three years of employment
