RN Case Manager (PRN) - East Valley (Scottsdale and Mesa/Chandler Areas)

Perform comprehensive patient assessments, develop individualized care plans, and administer medications, wound care, and other nursing treatments in a home setting. Monitor patient progress, modify care plans as needed, and educate patients and families on managing medical conditions and home care techniques. Document all visits and treatments in compliance with regulatory policies while advocating for patient needs through collaboration with interdisciplinary teams. Ensure patient safety, implement infection control practices, and provide emotional support to foster trusting relationships. Requires current Arizona RN license, two years of nursing experience including one year in home health, and a valid driver's license.
