Job Summary
RN Case Manager in Cohasset, MA, coordinating and managing care for SSH&EC clients; collaborates with interdisciplinary team; uses InterQual criteria to determine medical necessity for admission/continued stay; performs discharge planning and care coordination to optimize quality and cost-efficiency; identifies barriers to timely discharge; maintains knowledge of payer requirements and resources; requires RN licensure, CCM/ACM certification within two years, and Bachelor’s degree preferred; 3-5 years acute care experience preferred; schedule includes night/weekend shifts and a major holiday.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Bachelor's degree preferred
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
- Basic computer skills required
