RN Case Manager / per-diem

Job Summary
RN Case Manager provides clinically-based case management, discharge planning, and care coordination for SSH&EC clients; reviews medical records to ensure appropriate utilization and medical necessity using InterQual criteria; coordinates services and resources to facilitate cost-effective, high-quality patient outcomes; collaborates with interdisciplinary staff; identifies and refers cases for secondary review; communicates with attending physicians and payers to support timely discharge and appropriate level of care; maintains ongoing discharge planning and resource utilization, including documenting, coordinating referrals, and ensuring timely bed management; commits to working a minimum of 32 hours with at least 16 hours in night/weekend shifts over a 4-week schedule, plus one major holiday.
Required Qualifications
- RN - Registered Nurse
- Minimum 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Excellent verbal and written communication skills
- Knowledge of InterQual, EHR systems (Epic, Workday), and discharge planning processes
