RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates and advocates for complex patient care across the continuum, reviewing records to determine medical necessity for admission/continuation, performing discharge planning and care coordination, and collaborating with interdisciplinary teams to optimize quality and cost efficiency. Responsibilities include applying InterQual criteria, communicating with physicians about documentation and status changes, arranging appropriate services and referrals, and facilitating timely discharge. The role commits to a minimum of 32 hours with night or weekend shifts across a 4-week schedule and at least one major holiday. Requires RN licensure; ACM or CCM certification within two years of hire; Bachelor’s degree strongly preferred. Located in Pembroke, MA with on-site responsibilities.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Bachelor's degree preferred (strongly preferred)
- Minimum 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
