RN Case Manager / per-diem

RN Case Manager role (per-diem) focusing on clinically-based case management, discharge planning and care coordination for complex patients under Case Management supervision. Responsibilities include reviewing medical necessity for admission/continuation of care, coordinating services, negotiating with payors, facilitating safe discharge planning, and collaborating with interdisciplinary teams to optimize quality and cost-efficiency. Requires InterQual criteria knowledge, experience with utilization management, and ability to communicate with physicians, payers, and families. Schedule includes at least 32 hours with 16 hours across night/weekend shifts over a 4-week cycle and involves at least one major holiday; location is Brockton, MA with in-person duties. Licensure as an RN is required; CCM/ACM certification within two years of hire is required or preferred per the job listing; 3-5 years acute care experience and preferred CCU/ED experience are noted. Knowledge of healthcare financing, post-acute resources, and proficiency with EHR tools is expected.
