Job Summary
RN Case Manager provides patient advocacy, care coordination, discharge planning, and utilization management for SSH&EC clients. Coordinates, negotiates, and procures services to manage complex patient care, applying InterQual criteria to determine medical necessity for admission and continued stay. Works with interdisciplinary staff to ensure high-quality, cost-efficient outcomes, supports timely discharge planning, and assists in coordinating resources across the continuum of care. Requires strong negotiation and communication skills, critical thinking, and knowledge of healthcare financing, payer requirements, and post-acute care resources. Responsibilities include reviewing medical records, identifying cases for review, notifying physicians of coverage decisions, arranging prompt discharge, and facilitating appropriate level of care with attention to quality improvement and regulatory standards.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
