RN Case Manager / per-diem

RN Case Manager position responsible for clinically-based case management, discharge planning, and care coordination to ensure quality, cost-efficient patient outcomes. Collaborates with interdisciplinary staff, uses InterQual criteria for medical necessity determinations, and facilitates timely discharge and resource utilization across the continuum of care. Requires strong negotiation and communication skills, knowledge of healthcare financing, payer requirements, and post-acute care resources. Schedule includes a minimum 32 hours with night/weekend shifts and at least one major holiday; primary focus is hospital-based case management and utilization management.
