Job Summary
RN Case Manager position (per-diem) responsible for patient advocacy, coordinating and managing care for complex patients, applying InterQual criteria to determine medical necessity, and performing discharge planning to facilitate cost-effective, quality healthcare. The role collaborates with interdisciplinary teams, identifies barriers to timely discharge, communicates with physicians and payers, utilizes case management principles to optimize resource use and patient outcomes, and ensures compliance with regulatory requirements. Responsibilities include reviewing medical records for observation/admission status, coordinating care transitions, arranging referrals to post-acute services, and supporting documentation for reimbursement. Requires RN licensure, ACA/ CCM credentials within two years, and 3-5 years acute care experience; preferred qualifications include bachelor's degree and CCM/ACM certification.
Required Qualifications
- RN - Registered Nurse license
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
- Knowledge of InterQual criteria and utilization management
- Excellent verbal and written communication skills
- Knowledge of healthcare financing and post-acute resources
- Experience with EHR systems (e.g., Epic) and payer communications
- Ability to coordinate discharge planning and interdisciplinary collaboration
