Job Summary
RN Case Manager providing clinically-based case management and discharge planning, coordinating resources and utilization across the continuum of care for SSH&EC clients; uses InterQual criteria, negotiates services, ensures medical necessity and cost-effective quality care; collaborates with interdisciplinary staff to improve quality and efficiency; requires RN license and CCM/ACM certification within two years; minimum 32 hours with at least 16 hours in night and/or weekend shifts across a 4-week schedule; location is Kingston, MA.

- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
