RN Case Manager / per-diem

Job Summary
RN Case Manager to coordinate, negotiate, procure services for complex patients, oversee discharge planning and care coordination, and support utilization management to ensure cost-effective, high-quality care. Responsibilities include reviewing records for medical necessity, coordinating with physicians and payers, arranging timely discharges, and participating in quality improvement initiatives. Requires 32+ hours per week with 16 hours in night/weekend shifts per 4-week schedule and one major holiday; minimum 2 shifts must be night/weekend. Location: Scituate, MA (per-diem role in hospital).
Required Qualifications
- RN license
- CCM/ACM certification within two years of hire
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
- Bachelor's degree preferred
- InterQual knowledge
- case management experience
- Discharge planning and care coordination skills
- Excellent verbal and written communication
- Knowledge of healthcare financing and payers
- Proficiency with EHR systems (e.g., Epic) and utilization management tools
- Interdisciplinary collaboration experience
- Ability to multitask in high-stress environment
