RN Case Manager / per-diem

RN Case Manager position in Kingston, MA, per-diem/contract, supporting case management and discharge planning for SSH&EC clients. Responsible for reviewing medical records for observation/inpatient admissions to determine medical necessity, coordinating services across the continuum of care, and interfacing with multidisciplinary staff and payers to facilitate quality, cost-efficient outcomes. Responsibilities include applying InterQual criteria, notifying physicians of non-coverage decisions, arranging prompt and safe discharges, and actively monitoring patient status and documentation. Requires collaboration with physicians, nursing, admitting, and external insurance case managers; emphasizes utilization management, timely communication, and adherence to regulatory and organizational policies. Night/weekend shifts and at least one major holiday per 4-week schedule are required. Minimum education: RN with Bachelor’s-prepared preferred; 3-5 years acute care experience preferred; CCM/ACM certification within two years; strong knowledge of post-acute resources and EHR tools.
