RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates, negotiates, and procures services for complex patients, reviewing medical necessity for admission and continued stay, discharge planning, and care coordination to optimize quality and cost-effective healthcare. Works with interdisciplinary teams to ensure appropriate utilization, identify barriers to timely discharge, and facilitate transitions of care, including ensuring proper documentation, communicating with physicians, payers, and family, and supporting Medicare/insurance requirements. Requires RN licensure, case-management certification or ACM/ CCM readiness within two years, and a Bachelor's degree strongly preferred; 3-5 years acute care experience preferred, including critical care or emergency department exposure. The role includes night/weekend shifts totaling at least 32 hours in the 4-week schedule and participation in quality improvement initiatives. Licensure and certification requirements, payer knowledge, and proficiency with EHR/Interqual tools are emphasized.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Bachelor's degree preferred
- Knowledge of InterQual criteria
- Experience with EHR systems (e.g., Epic)
- Basic computer skills
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
