Job Summary
RN Case Manager coordinates, negotiates, procures services and resources for complex patients; provides clinically-based case management, discharge planning and care coordination to facilitate cost-effective quality healthcare; collaborates with interdisciplinary staff; reviews medical necessity for admission/continued stay using InterQual criteria; ensures timely discharge planning and resource utilization; commits to a minimum of 32 hours with at least 16 hours in night/weekend shifts per 4-week schedule and one major holiday.
Required Qualifications
- RN - Registered Nurse
- Bachelor's degree strongly preferred
