Job Summary
RN Case Manager coordinating care, advocating for SSH&EC clients, reviewing medical necessity and ensuring discharge planning, utilization management, and resource coordination across the continuum of care; collaborates with interdisciplinary team to optimize quality and cost-effective outcomes while guiding patients and families through care transitions.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Minimum 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
- Basic computer skills required
- Demonstrated negotiation, communication, conflict management, interdisciplinary collaboration, and critical thinking skills
