RN Case Manager / per-diem

RN Case Manager coordinates and manages care for SSH&EC clients under Case Management leadership, reviewing medical records for observation/admission status, applying InterQual criteria and payer requirements, and coordinating services across the continuum to optimize quality and cost efficiency. Responsibilities include discharge planning, care coordination, collaboration with interdisciplinary teams, interfacing with physicians and payers, identifying opportunities to reduce length of stay while ensuring appropriate utilization, and maintaining documentation and compliance with regulatory requirements. Requires licensed RN with CCM/ACM certification within two years, strong negotiation and communication skills, and experience in acute care/hospital settings. Minimum work commitment includes 32 hours with night/weekend shifts every 4 weeks and at least one major holiday.
