Job Summary
RN Case Manager position in Brockton, MA focused on coordinating, negotiating, and managing care for complex patients to achieve quality and cost-efficient outcomes. Responsible for discharge planning, utilization review using InterQual criteria, and interdisciplinary collaboration to optimize patient care and resource use. Requires clinical expertise in case management, knowledge of payer requirements, and strong communication across care teams. Provides 24/7 coverage with minimum 32 hours/week including night/weekend shifts, and participation in quality improvement activities.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- RN license required
- Bachelor’s degree strongly preferred
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
