RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates care for SSH&EC clients, performs discharge planning, utilization review using InterQual, and care coordination to facilitate cost-effective, quality healthcare. Works with interdisciplinary team, identifies and procures resources, monitors admission/continued stay criteria, and communicates with physicians and payers to support medical necessity and timely discharge. Requires an active RN license; CCM/ACM certification preferred within two years. Role involves 32-hour minimum with two shifts including nights/weekends over a 4-week cycle, and one major holiday per season. Location-based duties center on Cohasset, MA; on-site involvement with hospital care processes and patient flow management.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Bachelor's degree strongly preferred for education
- Minimum 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
