RN Case Manager / per-diem

RN Case Manager coordinates and advocates for patient care across the continuum, reviewing medical records to determine medical necessity for observation and admission, performing discharge planning and care coordination, identifying opportunities to reduce costs while maintaining high-quality care, and collaborating with interdisciplinary staff. Responsibilities include using InterQual criteria, communicating with physicians, facilitating timely discharge, arranging appropriate services or referrals, and supporting patients and families through the care process. The role involves nightly/weekend shifts (minimum 32 hours with at least 16 hours in 2 shifts during night and/or weekend per 4-week schedule) and participation in quality improvement activities. Skill emphasis includes negotiation, communication, interdisciplinary collaboration, critical thinking, and knowledge of payer requirements and post-acute resources. Tech tools referenced include EHR systems such as Epic, Workday, MCCM, and Interqual workflows.
