RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates patient care for complex cases, advocates for patients, and manages utilization and discharge planning to ensure quality, cost-efficient outcomes. Uses InterQual criteria, physician collaboration, and payor requirements to determine medical necessity for observation/admission/continued stays, facilitates timely discharge, and supports transitions across the care continuum. Works with interdisciplinary teams, documents and communicates with caregivers and payers, and maintains knowledge of payer requirements, post-discharge resources, and available community services. Requires RN licensure; 3-5 years acute care experience preferred; CCM/ACM certification within two years of hire preferred. Strong negotiation, communication, and critical thinking skills are essential; familiarity with EHRs (eDischarge, EHR, Interqual, MCCM, Epic, Workday) is expected. Minimum education typically RN with Bachelor’s prepared strongly preferred. Location: Brockton, MA; shift requirements include a minimum of 32 hours with at least 16 night/weekend hours over a 4-week schedule and involvement in a major holiday period.
