Job Summary
RN Case Manager coordinates and advocates for patient care across the continuum, reviewing medical records to determine medical necessity for admission/continuation, performing discharge planning and care coordination, partnering with physicians and payers to ensure appropriate resource utilization, and supporting timely, cost-effective patient outcomes with InterQual criteria and EHR tools; responsibilities include identifying barriers to discharge, arranging referrals, and communicating with multi-disciplinary teams to optimize patient flow and ensure compliant care plans.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
Desired Qualifications
- RN Case Manager experience
- InterQual utilization management
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Bachelor's degree strongly preferred
- Excellent communication and multitasking in high-stress environments
