RN Case Manager / per-diem

RN Case Manager coordinates, negotiates, procures services, and manages care for complex patients to achieve quality, cost-efficient outcomes. Works with interdisciplinary staff, participates in quality improvement, and applies InterQual criteria to determine medical necessity for admission and continued stay. Responsibilities include discharge planning, care coordination, and ensuring appropriate utilization of resources across the continuum of care. The role requires reviewing patient records, communicating with physicians, arranging prompt and safe discharge, and actively monitoring patients to support timely and appropriate level of care, with 32 hours minimum (including at least 16 hours in night and/or weekend shifts over a 4-week schedule) and one major holiday.
