RN Case Manager / per-diem

Job Summary
RN Case Manager provides patient advocacy and clinical case management to SSH&EC clients, coordinating, negotiating, procuring services and resources for complex patient care and discharge planning to support quality and cost-effective outcomes. Duties include reviewing medical records for observation/admission status using InterQual and payor criteria, determining medical necessity, coordinating with attending physicians and multidisciplinary teams, arranging safe discharge planning, and ensuring timely communication with payers, patients, and families. Requires collaboration across departments, participation in quality improvement activities, and flexible scheduling including night/weekend coverage; RN licensure and CCM/ACM certification (within two years) and 3-5 years acute care hospital experience preferred. Preferred Bachelor’s degree and Critical Care or Emergency Department experience.
