RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates care for SSH&EC clients, acts as patient advocate, negotiates and procures services, and manages care for complex patients to achieve quality and cost-efficient outcomes. Uses InterQual criteria to determine medical necessity for admission/continued stay, conducts discharge planning and care coordination across the continuum, collaborates with interdisciplinary staff, participates in quality improvement, and supports timely, appropriate utilization of resources. Responsibilities include reviewing medical records for observation/inpatient admissions, communicating with physicians to support accurate status and disposition, arranging prompt discharge planning, and ensuring documentation supports appropriate care transitions while leveraging payor guidelines and community resources. Requires strong negotiation, communication, conflict resolution, interdisciplinary collaboration, problem solving, and multitasking in a high-stress environment; maintains knowledge of payer requirements, post-acute resources, and utilization management processes. Licensure as RN with CCM/ACM certification within two years of hire; Bachelor’s degree strongly preferred. Locations: Brockton, MA. Minimum of 32 hours/week with night/weekend shifts and at least one major holiday per cycle.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Minimum Education: RN with Bachelor’s degree strongly preferred
- Preferred: Registered Nurse, Bachelors prepared
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
- Maintains RN licensure and Interqual certification
