RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates, negotiates, procures services and resources for, and manages care of complex patients to facilitate quality and cost-effective outcomes. Conducts clinical case management, discharge planning and care coordination, using InterQual criteria to determine medical necessity for admission and continued stay. Collaborates with interdisciplinary staff, supports quality improvement, and ensures timely discharge planning; maintains documentation and interfaces with payers to optimize reimbursement. Requires nursing licensure as RN with 3-5 years acute care experience preferred; CCM/ACM certification within two years of hire strongly supported; Epic/MCCM/EHR familiarity; able to work 32+ hours with night/weekend shifts as scheduled.
