RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates patient care for SSH&EC clients, reviewing medical records to determine medical necessity, conducting discharge planning, care coordination, and utilization management using InterQual and payer criteria. The role collaborates with interdisciplinary staff, identifies appropriate resources across the continuum, drives cost-efficient, high-quality care, and supports timely discharge planning. Responsibilities include communicating with physicians, payers, and care teams; arranging services, managing transitions across care settings, and ensuring appropriate documentation and reimbursement. Must commit to a minimum of 32 hours with two night/weekend shifts every 4 weeks and one major holiday.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Bachelor or higher degree preferred (BSN strongly preferred)
- Minimum 3-5 years acute care hospital experience preferred
Desired Qualifications
- ACM-Accredited Case Manager or CCM-Certified Case Manager
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
- Excellent verbal and written communication skills
- Knowledge of InterQual certification
- RN - Registered Nurse
- Basic computer skills
- Case management certification preferred (CCM/ACM within two years of hire)
- Bachelor's degree strongly preferred
