Job Summary
RN Case Manager coordinates and delivers clinically-based case management, discharge planning, and care coordination for SSH&EC clients under Case Management supervision. Responsibilities include reviewing medical records for observation/inpatient admissions to determine medical necessity using InterQual criteria, identifying cases needing secondary review, informing patients/families about insurance coverage changes, arranging prompt discharge planning, coordinating with physicians, nursing staff, and external payers, and supporting resource utilization across the continuum of care. The role emphasizes collaboration with multidisciplinary teams, participation in quality improvement, and maintaining up-to-date knowledge of payer requirements, community resources, and clinical tools (eDischarge, EHR, InterQual, MCCM, Epic, Workday). The position requires a nursing license and certifications in case management within two years of hire, with a minimum of 32 hours per week including night and/or weekend shifts and one major holiday per season.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
Desired Qualifications
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Bachelor's degree in Nursing (BSN) preferred
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
- RN licensure required
