Job Summary
RN Case Manager performs clinical, discharge planning, and care-coordination responsibilities to facilitate cost-effective, high-quality patient outcomes. Works with interdisciplinary teams to determine medical necessity for admissions and continued stays, coordinates services, and supports transitions across the continuum of care. Requires active RN licensure and may require CCM/ACM certification within two years of hire; Bachelor's degree strongly preferred. Night/weekend shifts and holiday coverage are included in the schedule.

- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
