RN Case Manager / per-diem

RN Case Manager coordinating patient care and resources across the care continuum, performing discharge planning, utilization review, and care coordination to optimize quality and cost efficiency. Responsibilities include reviewing records for medical necessity, communicating with physicians, arranging referrals and services, and collaborating with multidisciplinary teams to facilitate timely discharges, identify appropriate post-acute care, and ensure appropriate utilization of resources. Requires strong negotiation, communication, and problem-solving skills; knowledge of InterQual criteria, payer processes, EHR tools (eDischarge, EHR, InterQual, MCCM, Epic, Workday), and adherence to regulatory standards. Minimum education is RN with Bachelor’s prepared strongly preferred; required licensure is RN; CCM/ACM certification within two years of hire is expected. Location: Duxbury, MA, USA.
