RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates care for SSH&EC clients, reviews medical records for observation/admission status using InterQual and payor criteria, assists physicians with determining medical necessity, and facilitates discharge planning and resource coordination to optimize quality and cost-effective care. The role collaborates with interdisciplinary staff, participates in quality improvement, supports timely and appropriate utilization of services, and navigates payer requirements to ensure appropriate reimbursement. Requires RN licensure and case management certifications (ACM/CCM within 2 years), with 3-5 years acute care experience preferred; Bachelor’s degree strongly preferred.
Required Qualifications
- RN - Registered Nurse
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Minimum Education: Bachelor’s degree preferred (BSN strongly preferred)
- Minimum Work Experience: 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
