RN Case Manager / per-diem

RN Case Manager responsible for coordinating, negotiating, and managing care for complex patients, including discharge planning, utilization review, and care coordination across the continuum. Applies InterQual and payer criteria to determine medical necessity for admission and continued stay, collaborates with interdisciplinary teams, and supports cost-effective, high-quality care. Requires strong negotiation, communications, and problem-solving skills; maintains knowledge of payer resources, post-acute care options, and utilization management. Minimum education includes RN licensure with preferred bachelor’s-level preparation; 3-5 years acute care experience preferred, with CCM/ACM certification within two years of hire. The role involves reviewing records, coordinating transitions, and engaging with payers and physicians to optimize discharge planning and resource use. Locations: Scituate, MA, USA. Job type: CONTRACT; work includes night/weekend shifts as part of a 32-hour minimum with holiday coverage.
