RN Case Manager / per-diem

Job Summary
RN Case Manager coordinates, negotiates, and procures services for complex patients to support quality and cost-efficient outcomes. Acts as patient advocate, conducts utilization review for admission/continued stay using InterQual criteria, performs discharge planning and care coordination across the continuum, collaborates with interdisciplinary staff, and contributes to quality improvement. Requires active RN licensure; ACM/CCM certification within two years; 3-5 years acute care experience preferred. Minimum 32 hours weekly with two night/weekend shifts per cycle and observance of a major holiday. Hanover, MA-based; per-diem/contract engagement.
Required Qualifications
- RN licensure in good standing
- ACM or CCM certification within two years of hire
- Registered Nurse with Bachelor's degree preferred
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
