RN Case Manager / per-diem

RN Case Manager coordinates, negotiates, and procures services for SSH&EC clients under Case Management to ensure quality and cost-efficient patient outcomes. Responsible for clinical data review to determine medical necessity, discharge planning, and care coordination across the continuum, collaborating with interdisciplinary staff, and supporting utilization management with payors. Requires reviewing medical records, identifying cases not meeting criteria, notifying physicians, arranging timely discharge, and ensuring appropriate documentation. Minimum education includes RN licensure with a preference for a Bachelor's degree; CCM/ACM certification preferred within two years. Typical duties include using InterQual criteria, interfacing with physicians, arranging referrals to post-acute services, and contributing to quality improvement. Hours include a minimum of 32 hours with 16 hours across night or weekend shifts every 4 weeks plus coverage of a major holiday. Licensure, certification, and ongoing education in case management are emphasized, along with strong communication, negotiation, and multitasking abilities.
