RN Case Manager / per-diem

RN Case Manager provides clinically-based case management, discharge planning and care coordination to facilitate quality and cost-efficient patient outcomes. Collaborates with interdisciplinary staff to determine medical necessity for admission and continued stay using Interqual criteria, identifies and refers cases for secondary review, arranges prompt and safe discharge, and supports resource utilization across the continuum of care. Requires reviewing medical records, communicating with physicians, and coordinating with payers to ensure timely reimbursement. Maintains a focus on timely discharge planning, efficient care transitions, and adherence to regulatory requirements, while committing to a schedule that includes night and/or weekend shifts and a major holiday.
