Job Summary
RN Case Manager coordinating care for SSH&EC patients, performing clinical case management, discharge planning, and resource utilization across the continuum; collaborates with multidisciplinary teams to determine medical necessity for admissions/ongoing care, advocates for patients, negotiates services, and ensures timely, cost-effective, quality healthcare delivery; duties include reviewing records for observation/admission status, coordinating transitions to appropriate levels of care, managing documentation and communications with physicians, payers, and care providers, and supporting discharge planning and post-acute referrals; requires strong negotiation, communication, and critical thinking skills, and current nursing licensure.
Required Qualifications
- RN - Registered Nurse
- RN license required
- ACM-Accredited Case Manager or CCM-Certified Case Manager within two years of hire
- Bachelor's degree preferred
- 3-5 years acute care hospital experience preferred
- Critical Care or Emergency Department experience highly desirable
