RN-Case Manager

RN-Case Manager role responsible for assessing plans, coordinating, and evaluating home health patient care. Develops and implements Plan of Care based on clinical assessment, OASIS data collection, and utilization guidelines; uses clinical pathways and PPS concepts; supervises LPN and Home Health Aide; conducts admissions and discharge planning; maintains timely, accurate clinical notes and documentation; ensures safety and compliance with OSHA, JCAHO/CHAP, and state standards; requires Florida RN license, CPR, and ongoing CEUs. Location in Pensacola, FL with office/home care environment; duties include teaching nursing care, coordinating with physician and interdisciplinary team, and ensuring quality patient outcomes.
