Job Summary
Part-time RN Case Manager for Hospice in the Atlanta area. Responsible for coordinating home-based patient care, developing and updating individualized care plans per physician orders, and ensuring quality outcomes. Role includes patient/family education, completing OASIS and other records within timeliness guidelines, communicating changes in patient status to physicians and team, linking patients with social work and community resources, supervising LPNs and HHAs, participating in SOC/ROC and weekly team conferences, and delivering high-quality care in the home. Schedule is 3 days per week servicing Cobb, Douglas, Fulton and surrounding areas. Requires graduation from an accredited nursing program (Diploma, Associate, or BSN preferred), Georgia RN licensure, CPR, valid driver’s license, and home-health experience if PRN.
Required Qualifications
- Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred)
- Hold current licensure by the Georgia State Board of Nursing
- Valid CPR certification
- Possess a valid driver’s license and reliable transportation
- Have prior home health experience if applying as PRN staff
- Demonstrate knowledge of Medicare, Medicaid, and funding source regulations as applied to hospice/home health care
- Exhibit advanced clinical nursing skills and comfort collaborating with interdisciplinary teams
- Have proven decision-making, time management, and interpersonal skills, functioning independently in a home environment
- Proficient with computers and basic technology for documentation, email, HR systems, and online training
Desired Qualifications
- Experience coordinating home hospice/home health care

- Experience with OASIS documentation
- Strong communication with patients, families, and interdisciplinary teams
- Supervisory experience of LPNs and HHAs (per policy)
- Ability to manage caseload and optimize productivity
- Proficiency with computers and documentation systems
- Current Georgia nursing licensure and CPR certification
- Valid driver's license and reliable transportation
