Job Summary
Conduct comprehensive client assessments, including OASIS data collection within 48 hours of referral, and develop individualized Plans of Care incorporating current scientific findings. Provide skilled nursing interventions, administer medications, and submit all documentation and forms timely. Coordinate care and communicate with physicians, family, and the broader health team to report significant status changes and modify care plans as needed. Supervise applicable home care team members, assist with secondary chart reviews, and initiate referrals to other health professionals. Maintain client confidentiality, ensure infection control and environmental safety, and participate in mandatory education and team meetings. Position includes on-call coverage and requires travel to client homes and professional appointments daily.
Required Qualifications
- Graduate of an accredited school of professional nursing
- Current license as a Registered Nurse in the state of Connecticut
- Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair
- Good driving record
- Strong communication and interpersonal skills
- Position includes on call coverage

- Minimum of one (1) year experience in an acute care setting or equivalent experience
