Coordinate patient care across inpatient and outpatient settings for Parkridge Health Network. Assess patient needs, develop and manage care plans, and collaborate with physicians, interdisciplinary teams, patients, and families to ensure safe, efficient transitions of care. Monitor progress, advocate for resources, and promote quality, cost-effective outcomes.
