Job Summary
RN Case Manager in-home visits conducting weekly 1-2 hour assessments, providing direct patient care and coordinating overall care for former Nuclear Weapons Workers with chronic/terminal illness. Responsibilities include planning, implementing, and evaluating care; coordinating with regional director and staff; supervising and training clinical personnel; ensuring nursing documentation accuracy; participating in performance improvement activities; and supporting client care conferences and caregiver coordination. Requires RN licensure, state-approved nursing education, and prior home care experience; BSN preferred; CPR certification. Flexible scheduling with on-site work in Basin City, WA.
Required Qualifications
- Graduate of a state approved school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
- One (1) year of supervisory and/or case management experience preferred
- Current CPR certification
- BSN preferred
