Job Summary
RN Case Managers conduct in-home nursing visits for clients who are former Nuclear Weapons Workers with chronic/terminal illness. Responsibilities include direct care and case management, creating and evaluating care plans, coordinating with regional leadership and staff, supervising and training personnel, ensuring documentation accuracy, and participating in performance improvement initiatives. Visits are typically 1–2 hours weekly; drive time is compensated. Schedule is flexible to support personal and professional commitments; benefits available for full-time including PTO, medical/dental/vision, 401(k), and other voluntary benefits.
Required Qualifications
- Graduate of a state approved school of professional registered nursing
- Current, unrestricted RN license in the state(s) of practice
- Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
- Current CPR certification
