Job Summary
RN, Case Manager for PACE Program Float: responsible for assessing and identifying patient/family needs, coordinating the Plan of Care with the Interdisciplinary Team (IDT), and providing clinical, palliative and supportive care to keep the participant in their home environment. Requires Florida RN license and a minimum of 1 year nursing experience (hospice/hospital preferred). Duties include home visits, EMR/EHR documentation, care coordination, SBAR communications, and collaboration with IDT to address evolving patient needs. Must have valid driver’s license, reliable transportation, and ability to perform physical tasks and operate in home settings. BLS certification required; travel may be involved. Compensation range: $61,860.66 - $96,657.28 per year.
Required Qualifications
- Current license as RN in the state where the employee will be working
- Minimum of one year nursing experience; hospice or hospital experience preferred
- Certification of Alzheimer's Disease and Related Dementias Training (Florida Department of Elder Affairs) when working at PACE
- Experience with EMR/EHR system
- Mobile Driver - Valid driver’s license and automobile insurance per Company policy
- Reliable transportation to meet visit schedule
- Ability to use equipment with visual and auditory mechanisms
- Ability to communicate in English (verbal and written)
- Ability to visit participants in their homes for assessments
- Ability to perform essential functions and physical requirements (including lifting) with or without reasonable accommodation
- Active BLS for healthcare professionals (AHA or Red Cross)
