RN Case Manager - Ontario County

RN Case Manager will administer skilled nursing care to patients requiring professional nursing service, supervise family and nursing staff, and manage each patient’s plan of care in accordance with physician orders. Responsibilities include providing and coordinating quality patient care, maintaining a full caseload, documenting accurately, supervising Home Health Aides, conducting admissions and assessments, evaluating patient environments and home situations, interpreting nursing and agency services, ensuring regulatory compliance, infection control, and ongoing education for patients and caregivers. Must develop and implement the nursing care plan, deliver skilled nursing services and procedures, observe and report patient responses to treatments, and utilize computer-based documentation.
