Job Summary
RN Case Manager / Nurse Supervisor coordinating patient care in a home health setting across Southern Ohio. Manages a care team (RN, LPN, HHA, PCA) and oversees case management, SOC/ROC, wound care, lab draws, and discharge planning. Requires RN license in Ohio, CPR/First Aid, and at least 1 year of nursing supervisory or case-management experience. Travel involved across Highland County and surrounding counties with a flexible, part-time or PRN schedule (no overnight shifts). A defined growth path toward administrative leadership and QA/compliance roles is noted, with mileage reimbursement and benefits for full-time opportunities.
Required Qualifications
- Graduate of an accredited School of Nursing
- Minimum of 1 year experience as a Registered Nurse with active RN License in the State of Ohio
- CPR and First Aid Certifications (or obtainable prior to hire)
- 1 year experience in case management and home health; 2+ years home health preferred
- Supervisory experience in nursing; wound care and OASIS experience a plus
- Valid Driver's License and reliable transportation
- Ability to travel to surrounding counties
