Job Summary
Field-based RN Case Manager in Norwalk, CT responsible for home health visits across an assigned caseload in patients’ homes and facilities, performing head-to-toe assessments, medication administration, wound care per physician plan, developing the Plan of Care with other health professionals, utilizing Oasis assessments for admissions/re-certifications/discharges, educating patients and caregivers, and documenting via provided iPad; involves driving to patient sites and occasional weekend coverage.
Required Qualifications
- Registered Nurse Licensed in the state of employment
- graduate of an accredited RN program
- 1-2 years of RN experience (Home care preferred)
- Oasis experience strongly desired
- Experience in managing PleurX Catheters, Foley Catheters, Wound Vac Systems, and Ostomy care a plus
- Have reliable transportation and be willing to commute
- Some weekend days are required, and summer and winter holiday commitments as needed
- Basic knowledge of iOs operating system
