Job Summary
Coordinate all aspects of health care for individuals in residential homes by collaborating with treatment teams and Program Directors to review medical records, medications, and functional abilities. Supervise clinical operations, hire and train LPNs, and serve as a MAP coordinator while developing educational workshops and specialized protocols. Conduct comprehensive nursing assessments for admissions, develop case management and nursing care plans, and maintain documentation of health care encounters and monthly key data. Act as an advocate and liaison to ensure residents receive care consistent with clinical best practices. Drive to various locations for home visits and medical appointments.
Required Qualifications
- Must be fully licensed as a Registered Nurse in Massachusetts
- Must maintain licensure from the Massachusetts Board of Registration in Nursing
- Minimum three years nursing experience
- Three years experience in the direct care of the elderly or disabled adults
- Must drive to various locations for home visiting purposes and medical appointments
