RN Case Manager (Non-Supervisory)

Job Summary

Manage chronic illness, mental health, and life-altering cases by assessing physical and mental wellness, developing individualized care plans, and coordinating referrals for ambulatory, inpatient, post-acute, and community services. Triaging referrals within 24 hours to direct urgent care coordination, while collaborating with multidisciplinary teams to establish and adjust treatment plans based on patient progress. Document all interactions, referrals, and case reports in the NextGen EHR, evaluate outcomes against recognized guidelines, and assist families with financial assistance and eligibility determinations. This role may require covering RN clinical duties during staffing shortages and working weekends or evenings as needed.

Required Qualifications

  • Current California Registered Nurse license
  • Minimum of 3 years' experience in a similar healthcare role
  • Knowledge and understanding of HIPAA in the ambulatory care setting
  • Absolute ability to maintain confidentiality according to standards
  • Current CPR certification
  • Current ACLS certification
  • Valid driver's license
  • Ability to be insurable under SIHC vehicle insurance policy

  • Adequate health to perform all duties of the position
  • Must be able to accurately assess the criticality of need within 24 hours of referral
  • Must be able to direct Care Coordinator and/or the Patient Advocate to coordinate care for patient in urgent situations
  • Must possess current CPR; ACLS must be obtained in the first four weeks of employment
  • Must be able to lift up to 25 lbs
  • Must be available for weekends and evenings as needed
  • Must be enrolled, or be eligible for enrollment, as an American Indian with their Tribe, or must be certified as an American Indian from the designated Tribal Representative (for Indian Preference)
