RN Case Manager NE- Care Coordination- Per Diem

RN Case Manager per diem in a hospital-based care coordination setting. Under the supervision of the Director of Case Management, provides clinically-based case management to support effective and efficient patient care, collaborates with health care team to identify appropriate resource utilization and develop transition plans including community referrals, adheres to Medicare conditions when needed, and conducts utilization reviews. Requires a Bachelor's degree in a healthcare-related field (or equivalent experience) and 3–5 years of case management experience; CCM or ACM preferred; 8am–4:30pm shift; SolutionHealth/Elliot Health System affiliation in Manchester, NH.
