Job Summary
RN Case Manager responsible for coordinating and optimizing patient care across the continuum in a hospital setting. Conducts assessments, develops and implements care plans, collaborates with providers and community resources, advocates for patients and families, monitors progress, and ensures appropriate use of services. Supports safe, timely discharges and ongoing professional growth within a team-focused, interdisciplinary environment at Mohawk Valley Health System.
Required Qualifications
- Current NY RN license
- Experience in case management or care coordination (hospital/health system)
- Strong assessment and care-planning skills
